How do I go about getting an initial evaluation and what is the cost?
Patients are encouraged to send their clinical information in for an evaluation. We will need copies of your actual MRI films, plus results from any tests that relate to your condition, such as blood work or visual field exams. To ensure a thorough evaluation, we recommend that you consult with one of our nurses who will assist in collecting the needed information. Call toll free 866-266-9627
